Here is an example of a SOAP note for a patient with a viral infection:
Subjective:
The patient is a 25-year-old male who presents to the clinic with complaints of fever, sore throat, and fatigue. He reports that he has been feeling this way for several days and that his symptoms have been getting progressively worse. He also reports a dry cough and muscle aches.
Objective:
On examination, the patient appears ill and has a fever. He has swollen lymph nodes in his neck and a red, inflamed throat. Vital signs are otherwise within normal limits. Laboratory tests may be ordered to confirm the presence of a viral infection.
Assessment:
The patient is suspected to have a viral infection, with symptoms of fever, sore throat, dry cough, fatigue, and muscle aches.
Plan:
The patient will be advised to rest, drink plenty of fluids, and take over-the-counter medications such as acetaminophen or ibuprofen to relieve fever and pain. Symptomatic relief may also be provided for sore throat and cough. Antiviral medications may be considered depending on the specific virus identified. The patient will be advised to practice good respiratory hygiene, such as covering his mouth when coughing or sneezing, and to avoid close contact with others to prevent the spread of the virus. Follow-up appointments will be scheduled to monitor the patient’s response to treatment and adjust the treatment plan as needed. The patient will be advised to report any new symptoms or changes in his condition to his healthcare provider immediately.
