Here is an example of a SOAP note for a patient with rheumatoid arthritis:
Subjective:
The patient is a 45-year-old female who presents to the clinic complaining of joint pain and stiffness. She reports that the pain is worse in the morning and improves throughout the day. She has also been experiencing fatigue and difficulty sleeping due to the pain.
Objective:
On examination, the patient has swelling and tenderness in multiple joints, including the wrists, fingers, and knees. She has limited range of motion in these joints and is experiencing joint deformities. Laboratory tests show elevated levels of rheumatoid factor and anti-cyclic citrullinated peptide (anti-CCP) antibodies.
Assessment:
The patient has rheumatoid arthritis with involvement of multiple joints, joint deformities, and elevated levels of rheumatoid factor and anti-CCP antibodies. Her symptoms suggest active disease with significant impact on her daily activities and quality of life.
Plan:
The patient will be prescribed disease-modifying anti-rheumatic drugs (DMARDs) to help slow the progression of the disease and relieve her symptoms. She will also be advised to make lifestyle modifications, including regular exercise and a healthy diet, to support her overall health and well-being. Follow-up appointments will be scheduled to monitor her response to treatment and adjust the treatment plan as needed. She will also be referred to a physical therapist to help improve her range of motion and strength in her affected joints.
