Subjective:
The patient is a 35-year-old female who presents to the clinic with complaints of a rash on her arms and legs that has been present for several weeks. She reports that the rash is itchy and occasionally painful, and that it has been spreading gradually. She denies any recent changes to her diet, medication, or environment.
Objective:
On examination, the patient has a widespread rash on her arms and legs that appears to be erythematous and scaly. There are no obvious lesions or vesicles, and the rash appears to be symmetrical. A skin biopsy may be ordered to confirm the diagnosis.
Assessment:
The patient has a rash consistent with a dermatological condition, with symptoms of itchiness, pain, and spreading.
Plan:
The patient will be referred to a dermatologist for further evaluation and management of her condition. A skin biopsy may be ordered to confirm the diagnosis. Symptomatic relief may be provided for the patient’s itchiness and pain, such as topical creams or oral antihistamines. The patient will be advised to avoid any triggers that may exacerbate her symptoms, such as certain fabrics or soaps. Follow-up appointments will be scheduled to monitor the patient’s response to treatment and adjust the treatment plan as needed. The patient will be advised to report any new symptoms or changes in her condition to her healthcare provider immediately.
